    Payroll Direct Deposit Form

    The Town of Sutton offers two types of Direct Deposits.  You can enroll in one or both:

    Fixed Dollar Amount(s):  This Direct Deposit is to have a fixed dollar amount deducted from your paycheck to be directly deposited to a banking institution(s).  The dollar amount can be deposited into a checking or savings account.

    Percentage of Net Pay:  This Direct Deposit is to have a percentage(s) of your net pay (after all taxes and other deductions and/or fixed amount direct deposits) directly deposited into a banking institution(s).  The percentage(s) of net pay can be deposited into a checking or savings account.  If you wish have additional percentages of your net pay dispursed, please fill out another Direct Deposit Form(s).  However, all net pay deposits must add up to 100%.

    NOTE:  If any Direct Deposits are going into a checking account, please attach a blank, voided check to the form.  If any Direct Deposits are going into a savings account, please provide a form from your financial institution indicating the routing and account number.

    Form W-4 (Federal Employee's Withholding Form)

    Fill out, sign and date.  Submit completed form to Gail Woodland in the Business Office.  The change you make does not automatically update in the payroll system.

    Form M-4 (Massachusetts Employee's Withholding Exemption Certificate)

    Fill out, sign and date.  Submit completed form to Gail Woodland in the Business Office.  The change you make does not automatically update in the payroll system.
