Student Work Permit
If you are seeking employment and are under the age of 18, you will need to complete the work permit process prior to starting work. Please see the instructions below for information on how to complete the process. Before you begin the work permit process, please review the "Summary of Massachusetts Laws Regulating Minorsโ Work Hours and Occupation Restrictions"
1. Securing Employment
To begin the process, you will first need to secure employment. If you haven't been offered a position yet or are still in the interview process, you will need to wait until you have been offered a job in order to complete the form.
2. Completing the Work Permit Application
Once you secure a job, you can complete the Work Permit Application. Please bring the form to your employer and they will complete the first page. On the second page of the application, you and your parent or guardian will sign. Each of these signatures are required. If you are 14 or 15 years old, you will also have to obtain your physician's signature. **Please note proof of age is required, such as photocopy of your birth certificate, passport, or immigration record.**
3. Submitting the Work Permit Application
Once your Work Permit Application is complete in its entirety, bring your application and proof of age document to the high school Guidance office. Once your work permit is completed, bring it to your employer and begin your job. Your work permit is ONLY good for the employer listed on your application. If you obtain a new job, you will need an entirely new permit. Your permit will expire when you are 18 years of age.
Please note that if you do not currently attend Sutton Memorial High School, you will also need to bring the following information:

Current school
Current grade
Length of enrollment is current school
